If you believe ProHealth failed to provide these services, or discriminated in any way, you may file a grievance with our Civil Rights Coordinator. We will not retaliate against you for filing.
You may file in person, by mail, by phone, by fax or by email. If you need help filing, our Coordinator will help you. We will acknowledge your grievance within 5 business days and respond in writing within 30 days.
